New Patient Experience

Your first visit here truly is an experience and we hope you'll agree that it's the most thorough dental examination you've ever received.

The most important part of the New Patient Experience is finding out your specific needs and questions. Are you interested in whitening your teeth? Are you curious about veneers? Maybe you're nervous about visiting the dentist and want to know about the things we can do to alleviate your anxiety. Perhaps you have a missing tooth and want to know your options to fill in the space.

Whatever your concerns, we will develop a custom plan that meets your dental needs. 

We will gather information about you so that you may be thoroughly treated. Your experience will include:

• A review of your medical and dental history.
• Having necessary x-rays taken and discussed with you.
• A complete dental health evaluation and risk assessment completed in four main areas:
• Gum and Bone
• Tooth Support
• TMJ, Bite and Chewing
• Smile
• Digital photos will be taken and shared with you.
• Most importantly, you have the opportunity to quietly sit and talk with the doctor about your dental health and be a part of the team that designs your dental treatment plan.

To facilitate being seen as soon as possible at the time of your appointment, please download and complete the following forms. Please assist us by providing the following information at the time of your consultation:

• Completed Patient Information Packet.
• A list of your current medical prescriptions.
• X-rays: If your previous dentist has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at Madison Dental Group.
• If you have dental insurance, bring your insurance card.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
